Professor Di Mcintyre

Di McIntyre is the South African Research Chair in ‘Health and Wealth’ and a Professor in the School of Public Health and Family Medicine at the University of Cape Town, where she has worked since 1988. She founded the Health Economics Unit (HEU) in 1990 and was its Director for 13 years. She has completed Bachelor of Commerce, Honours and Masters degrees in Economics at the University of Cape Town. She has also completed a PhD by dissertation, entitled “Health care financing and expenditure in South Africa: Towards equity and efficiency in policy making”.

In addition to general technical support activities in South Africa and other African countries, she has served on a number of policy committees. These include serving on the Health Care Finance Advisory Committee to the Department of Health of the Government of National Unity, the Minister of Health’s Committee of Inquiry into a National Health Insurance for South Africa and the Medical Schemes Council. She also chaired the Minister of Health’s Medicine Pricing Committee since its establishment in 2003 until 2008.

Research interests: Health service financing, equity in the health sector, access to and utilisation of health services, geographic allocation of health care resources, the role of the private health sector, and public/private sector mix issues

Current research projects: Current involvement in HEU research projects includes:

  • Evaluation of alternative sources of finance for improved provision of health services. Specific areas of interest include critical evaluation of mandatory health insurance, tax funding and fiscal space issues, and the consequences of user fees at public sector health facilities. She is the principal investigator and co-ordinator of a multi-country (Ghana, South Africa, Tanzania) research project evaluating the potential role of mandatory health insurance in promoting equitable financing and benefit incidence in these countries.
  • Equity issues within the health sector, especially in terms of the distribution of health and health care resources between different socio-economic groups. This includes identifying the key determinants of health and health system inequity. Household level economic and social consequences of illness, and strategies for coping with these consequences, is another area of equity related research. Much of this work is based on the analysis of recent household survey databases. Another component of this equity work is the evaluation of the impact of health sector reform initiatives on poverty and inequality.
  • Access to and utilisation of health services. This research interest focuses on exploring equity of health care utilisation patterns and the distribution of the benefit of health care use relative to the need for health care across socio-economic groups. It also considers methods for measuring and analysing three core dimensions of access: availability, affordability and acceptability of health services.
  • Evaluation of the geographic allocation of health care resources. This focuses on the analysis of current disparities in resource availability and mechanisms for achieving greater equity in the distribution of resources, including the investigation of alternative resource allocation formulae. This research considers both distribution from national to provincial level, and from provincial to district level. It also focuses on mechanisms for promoting equitable health care resource allocation within a fiscal federal context.
  • The role of the private health sector, and public/private sector mix issues. Recent research evaluates the equity of the public/private health mix in relation to financing and service provision and identifying mechanisms promoting equity in the public/private mix.

Current teaching: Her training activities include participation in a health economics research internship programme, presenting specialised training courses for public and private sector health managers, and training on all of the HEU’s postgraduate courses (Postgraduate Diploma, Masters and PhD programs).
